Parent/Guardian Consent

As Parent or Guardian of the Participant, I certify that he/she is under my guardianship and has my permission to participate in the Golden Spike Days Festival Tri-Cities' Got Talent Auditions and Live Performance and you have approved their registration and acceptance to perform and audition in the Tri-Cities' Got Talent contest. In consideration of the undersigned being permitted to participate in any of the events carried on, sanctioned and/or sponsored by the Port Moody Golden Spike Days Society (the “Society”) or situated upon the grounds or utilizing any facilities of the Golden Spike Days, if for myself and/or on behalf of any child or ward of mine under the age of majority, I do hereby release their officers, directors, employees, agents, officials, servants, organizers and representatives from and against all claims, actions, costs, expenses and demands whatsoever concerning death, injury, loss or damage to me or any child, in respect of which I am a guardian or ward of under the age of majority, by virtue of my, his or her participation in said events, however caused and, regardless of whether same may have been contributed to or occasioned by the negligence of the said bodies, or any of them, their agents, organizers, officials, directors, employees, servants or representatives and each of the last mentioned parties shall be deemed to be a party to this agreement as if the Port Moody Golden Spike Days Society were acting as each party's agent or trustee.

Video, Photo & Social Media Release Form

The Port Moody Golden Spike Days Society, publishes printed material, prepares power point presentations, updates its website (goldenspike.ca) and engages in general marketing and public relations purposes that may use photographs of people attending events, attractions or functions on its site. The photographs, videos, and audio impressions taken on its Rocky Point site, including auditions videos, may appear in local newspapers, on our website or in other publications including social media sites, live or recorded. We would like your permission to use any photos, videos and audio impressions that may have been taken prior, during or after your performance with us. It is understood and agreed that this agreement is to be binding on myself, my heirs, executors and assigns and that I have read and fully comprehend this agreement.

As Parent or Guardian or the Participant over 18 years old, I have read and understood this Port Moody Golden Spike Days Society: Tri-Cities' Got Talent Contest Parent/Guardian Consent & Photo, Video & Social Media Release Forms. I agree that I participate voluntarily in the program upon the basis of its terms.

Children under 19 years old must have a parent or legal guardian sign this Consent & Release Form on their behalf.
